Please note: Initial assessments begin at $690 for people who are self-funding their assessment. This is a simplified assessment to reduce cost. 

*** Uploading a copy of the NDIS plan is not mandatory. However, will assist the team in completing this assessment.

Please note this referral is valid for 6 months. A copy of the report will be sent to you after completion of the assessment.
